Home visits from a nurse are available to residents of Dickey County through the Adult Health Maintenance Program. These visits can be as frequent as one time per week or as infrequent as every two months. One of the goals of this program is to provide services that will assist individuals in remaining safely in their own homes for as along as possible.
Evaluation visit in your home upon request of you, your family, or your physician to assess if we can provide services which will enable you to stay in your home.
*Assessment of the individual’s physical status.
*Medication set-up and monitoring.
*Health education and support.
We work on a suggested fee schedule in our office. Medicaid will be billed for nursing home visits.
